This form needs to be completed if your student plans to ride the bus to/from school and/or for athletics.

Once again, we will be providing FREE breakfast and lunch for ALL students. Please help the District by completing the Household Income Report form below. Only one form is needed per household.

All Middle School and High School athletes are required to have a new physical every year BEFORE they can attend practice with their team. Athletes must also complete an annual Athletic Agreement.

All student drivers must complete both portions of the Student Driver Agreement and register their vehicles with the High School office.

If your student requires medication to be administered during the school day, the following Medication Form is required. Provide Medication Form and medications to the school office. Students are not to bring medications to/from the school. All medications must be brought in by a parent or guardian.

The Chippewa Hills Wellness Center is a school-based health center for youth and young adults ages 5-21 attending school at Chippewa Hills School District. The Wellness Center is operated through District Health Department #10 and offers both medical and mental health services. In order for students to receive treatment at the Wellness Center, the Parent/Guardian/Client Consent Form is required. The Consent form can either be downloaded and printed below or you may fill out the online consent form.

All volunteers must have a criminal background check done before working in our school. Please fill out the volunteer form below and turn it in to your school office to begin the background check and fingerprinting process. Thank you for taking the time to work with our Warriors!
