Chippewa Hills Athletic Agreement

Participation in athletics is a privilege and all Chippewa Hills athletes are expected to live up to high standards in representing themselves, their family, the school, and the community. The contents of this handbook help define the policies and procedures used by the Athletics Department that are specific to the athletes and their parents and guardians.

All Chippewa Hills athletes and their parents must sign a new Athletic Agreement every school year. The Athletic Agreement is found on the last page of the handbook.

MHSAA Physical Form

All Michigan high school and middle school athletes MUST complete a physical exam by a Certified Health Professional (MD, DO, PA, CNP) before the athlete can begin practice with the team. The athlete's physician and parent/guardian must sign the MHSAA Physical Form, including the medical treatment consent, parent or guardian consent, and emergency information. A new physical is required for every academic year and must be completed after April 15th of the previous school year.

Athletic Hardship Application

Scholarship funds are available for athletes struggling to pay their athletic fee. Those who wish to apply must submit the Hardship Application below. As a guideline for assistance, we will use household income requirements for the Federal Free and Reduced Meal Assistance Program. If you qualify for assistance, you may still be asked to pay a small portion of the athletic fee as well as volunteer to work at one athletic event to support our event staff.

MHSAA Post-Concussion Return to Activity Form

This form is to be used after an athlete has been removed from activity after exhibiting concussion symptoms. MHSAA rules require written authorization from a physician granting permission for the athlete to return to unrestricted activity.
